General Information
Enlightened Ways is a bodywork office located in Leesburg, Florida offering Neuromuscular Therapy/Deep Tissue, Swedish, Lymphatic, Prenatal, Reiki and Wellness Coaching. Our hours of operation are from 10-6 Sunday through Thursday. Contact hours are between 10-8:30 on those days. Once you are fully booked you will receive a confirmation text with information on how to locate the office.
Client Intake Form & Waiver of Liability
Before your appointment, you will need to fill out a Client Intake Form and Liability Waiver. You can fill out the one below and either email it directly or print it out for your appointment. I will also have one available when you arrive.
The Appointment….
First off, please arrive on time for your appointment. If you are going to be late for any reason, please call or text to let me know. If you are going to be more than 10 minutes late, we will need to reschedule the appointment. Cancellations must be done 24 hours in advance. If you are late beyond 10 minutes and you haven’t called or texted this will be considered a no call/no show. A no call/no show will result in the full price being charged for the appointment before you can be scheduled again.
